A long-term partner for lasting change.
WORK WITH USHandoff failure isn't a one-time fix. It's a process of changing the culture behind it. Each step builds on what came before: your data, your context, your team's reality. We don't offer a menu of services. We're inviting you on a journey, one we run together.
WHO WE ARECheo Rose-Washington
I started Hand-Off because a failed handoff nearly cost me my ability to walk. Miscommunication between two hospitals led to one of the rarest injuries in medical literature: two bilateral patellar tendon ruptures within two weeks. I went from patient to founder because I wanted to understand why it happened, and I discovered that the answer isn't what most people think. It's not a tools problem. It's a people problem. That's what I've been working on ever since.
Jessica Snitko, Ph.D.
I spent years studying how people share and understand complex information, first at Purdue University where I earned my Ph.D., then as a professor and a trial consultant on medical malpractice cases. I kept seeing the same pattern: simple communication errors with catastrophic consequences. When Cheo showed me what he was building, I knew this was the work I'd been training for. I left academia to go all in.

HOW WE WORK1. Readiness Report™
Your RRRT data, interpreted at the organizational level. Individual scores are useful. But when your whole team participates, we can show you something bigger: where perceptions diverge across roles, which of the five elements breaks down most, and how your organization compares to other organizations. We deliver it all in a detailed written report.
2. Hand-Off Culture Diagnostic™
The why behind your numbers. Building on your Readiness Report, we add The Glue Principle™: the incentive and cultural forces that determine whether your team follows through or just checks the box. Includes demographic analysis across your organization and a virtual debrief with our team.
3. Quality Improvement Partnership
From findings to action. We translate your diagnostic results into a prioritized plan: what to fix first, how to fix it, and how to measure whether it's working. Every partnership is tailored to your organization.
“Cheo’s HFES presentation was a powerful reminder that patient handoffs are not simply operational steps, but critical moments of vulnerability in care. By sharing his lived experience following an inter-institutional communication failure, he brought human urgency and clarity to challenges that too often remain abstract.
His perspective connected patient, provider, and system realities in a way that underscored our shared responsibility to design safer, more reliable care transitions. The talk challenged us to move beyond incremental fixes and rethink how we approach handoff design through a true patient-safety lens.”
Hakim Lakhani, Senior Director, Health Systems Transformation at Dana-Farber Cancer Institute
